Med spa marketing agency: paid media that books consultations

Plaid Testing is a med spa marketing agency for medical spas and aesthetics groups spending $50K or more a month on ads. We run Google, Meta and TikTok to booked consultations and tracked treatment revenue, not clicks, under a fractional CMO who owns the strategy. Based in Orange County, working with practices across the US.

The med spa market rewards the practices that can measure

There were 10,488 medical spas in the US in 2023, up from 8,899 the year before, and 18 percent of them had opened within the previous twelve months, according to AmSpa’s 2024 State of the Industry report. Average revenue per practice sat near $1.4 million, average spend per visit was $527, and 73 percent of patients were repeat patients. AmSpa put the industry above $17 billion in revenue in 2026, growing by more than $1 billion a year.

Two things follow for paid media. First, competition for the same consultation is rising faster than the patient pool in most metros. Second, the economics are a retention business: a patient won once is worth many visits, so the right cost per booked consultation is far higher than most practices allow themselves to pay, and far lower than most agencies report once no-shows are counted. Our job is to put the real number on both.

What a med spa marketing agency should run

ChannelThe med spa constraintWhat we run
Google Search and Local ServicesHighest intent in the category: “botox near me”, “lip filler [city]”, “laser hair removal cost”. Expensive, and wasted without call and booking tracking.Campaigns by treatment and location, call tracking and booking events as conversions, brand carved out, Local Services Ads where eligible, budget by treatment payback. Google Ads.
Meta and InstagramAesthetics creative sits under Meta’s health policy: before and after imagery and treatment claims get rejected and can classify the account.Lead and booking campaigns with server-side tracking, creative built on the experience and the practitioner rather than transformation, and retargeting that fills the consultation calendar. Meta Ads.
TikTokStrong for discovery with 18 to 34 year old patients, the segment AmSpa shows growing in share of female patients.Spark Ads on practitioner and patient content, tested in month two, scaled on booked consultations. TikTok Ads.
TrackingThe sale happens in the practice management system, not online. Most agencies report leads; the practice needs revenue.Booking platform and practice management events connected to ad platforms, no-show and conversion-to-treatment rates reported next to cost per lead, and revenue per new patient as the scorecard. Tracking and attribution.
Creative testingCompliance and taste both matter; the practice’s reputation is in the ad.A weekly test cadence with a compliance pass, hooks and offers tested in small sandbox campaigns before scale. Performance creative.

We do not run SEO, review management or email and SMS for practices. We have partners who do.

Where med spa paid media breaks

1. Leads are counted, consultations are not

A form fill is not a patient. Between the click and the chair sit a reply time, a booking, a show rate and a conversion to treatment, and each one leaks. We report cost per booked consultation and cost per treated patient, which means the practice’s front desk response time is part of the media plan, not someone else’s problem.

2. Every treatment gets the same budget

Injectables, laser, body contouring and skin treatments have different margins, different repeat rates and different ad costs. A single “book now” campaign spends the laser budget on injectable patients. We build by treatment and set targets from each treatment’s payback, including the repeat visits that make up 73 percent of the patient base.

Med spa ad account structure by treatment: injectables, laser, body contouring, skin and facials

3. The creative that converts is the creative that gets rejected

Before and after imagery and outcome claims convert, and they are what Meta’s health policy flags. The practices that keep scaling on Meta in 2026 are the ones that moved to practitioner-led, experience-led and education-led creative before enforcement forced them to.

4. One location’s results hide another’s

For multi-location groups, blended reporting hides a location that is losing money. We report by location from day one and move budget to where the consultations are cheapest and the chairs have capacity.

How we run med spa accounts

  1. Audit (free, 30 minutes). Are bookings and calls tracked as conversions? What is the real cost per booked consultation and per treated patient? Where does the structure leak budget? Three fixes, whether or not we work together.
  2. Rebuild (first 30 days). Booking and call tracking connected to Google and Meta, server-side events, campaigns split by treatment and location, creative pipeline with a compliance pass, daily reporting live.
  3. Scale. Daily snapshot, weekly tests, monthly strategy. Budget follows cost per treated patient and chair capacity, not platform ROAS.

Proof from the work

We publish client figures only with written permission and only when they trace to a platform export we hold. Practice results are walked through on the audit call rather than printed here. What we publish is the operating system, verified in Triple Whale on three ecommerce accounts: sales to $9.27M in the first half of 2026 on one, net margin from 32.8 percent to 48.3 percent on another while ad spend rose 419 percent, and new-customer CPA from $86 to $67 on a third. See the case studies. For a practice the sale moves from the cart to the consultation, which is why booking integration is the first thing we build. What this does not claim: no incrementality test was run, blended figures come from Triple Whale, and product and merchandising moved these numbers alongside media.

Who this is for

  • Medical spas and aesthetics groups spending $50K or more a month on ads, single or multi-location.
  • Practices with a booking platform and the willingness to connect it to the ad accounts.
  • Owners who want cost per treated patient, by treatment and location, on a daily report.

Not for single-location practices under $25K a month in ad spend, or practices looking for SEO, reputation management or social media management as the main service.

What it costs

No retainer is required to start. Pricing depends on monthly spend, locations and whether the work is done for you or with your team. Our guide to how much marketing agencies charge in 2026 explains the models.

Questions med spa owners ask us

What is a good cost per lead for a med spa?

Cost per lead is the wrong number. The number that matters is cost per booked consultation, then cost per treated patient, and both depend on the treatment. A $150 cost per booked consultation can be excellent for a body contouring patient worth thousands over two years and poor for a single facial. We set the target by treatment payback.

Can med spas still advertise on Meta in 2026?

Yes. Meta’s health and wellness policy restricts outcome claims and before and after imagery and can limit optimization events for classified accounts. It does not stop a compliant practice from advertising. The creative and the tracking have to be built for the policy.

Do you work with multi-location groups?

Yes, and we report by location from the first day so budget can follow capacity. The 2024 AmSpa report put the average multi-location group at nine locations, which is exactly where blended reporting starts to hide problems.

Are you an ecommerce agency or a med spa agency?

Both, and we say so. Our deepest work is in apparel and supplement ecommerce, where the whole sale happens online. Med spa paid media uses the same tracking discipline with the sale moved into the practice, which is why booking integration is the first thing we build.
